Select Permanent if:

You have no plans to move back to your old address. (Most moves are permanent)

Choose temporary if you plan to move back to your old address in less than six months.

You can request forwarding your mail to your temporary address for a minimum of two weeks and a maximum of six months. You can extend your temporary forwarding as long as the total forwarding time is less than 12 months.

Please note that if you plan to move to this new address for less than six months and then to a different address (not the one that you are moving from), do not check the Temporary box or provide a stop forwarding date. You will simply need to fill out another Change of Address request at that time.

Individual:

Select this option if only one person is moving or if the individuals moving have different last names (which will require separate Change of Address requests).

If you receive mail by more than one name (maiden name, married name or nickname), select "individual" and fill out a separate form for each name.

Family:

Select this option if your entire family has the same last name and is moving to the same address

If anyone with the same last name is staying at the old address, select the "individual" option. In all cases, family members with different last names or those family members moving to different addresses must prepare separate forms.

Business:

Select this option if a business is moving. An individual change of address from a business address is not acceptable.

The company remaining at the address is responsible for forwarding any mail that belongs to you and not the company.

First Name (or Initial)

- If you selected INDIVIDUAL, enter your FIRST NAME or initial.
- If you selected ENTIRE FAMILY, enter the first name or initial of the head of the household.

Middle Name (or Initial)

- Fill in your MIDDLE NAME or INITIAL.

- If you selected ENTIRE FAMILY, enter the middle name or initials of the head of the household.

Last Name

- Fill in only one LAST NAME.

- If anyone with the same last name is moving to a different address, use a separate form for each person.

Note: If you receive mail by more than one name (maiden name, married name, or nickname) please fill out a separate form for each name and check the INDIVIDUAL box
